Social prescribing (SP) is an emerging framework with an evolving definition. Recognizing the healthy and vibrant diversity of models, this interactive map specifically features initiatives that embody what we consider to be foundational principles of SP. These pillars include:


  • A supportive bridge between healthcare and non-clinical, community-based supports and services;
  • A person-centred approach where solutions are co-created based on what matters to clients;
  • Intentional follow-up and engagement with clients, health providers, and social and community service organizations;
  • Data collection and tracking.

The Social Prescribing Initiatives Map is powered by Caredove, an access management platform that creates collections of home and community services. Creating a free Caredove account will enable you to update your organization’s information on this map, but it is not required. To learn more about Caredove and how it might support your organizational needs, or for Caredove technical support in creating your account, follow the steps below.


  1. Fill out the form with your basic initiative information.
  2. Meet with Kay, the Caredove Operations expert who will meet with you virtually to get you familiar with Caredove.
  3. Learn how to update your social prescribing service information, visible on the CISP website.
  4. Optional: Learn how to streamline service requests right from your website, including create and editing service listings, building intake appointment schedules and managing your inbound referrals.


If you are already using Caredove to receive referrals, you may not require any additional training. Kay will be in touch and answer any questions you may have.
