Quite simply, to provide better access to community services and healthcare.
As a trusted partner of the Archipel Ontario Health Team (OHT), you've been invited to join the Archipel OHT Navigation & Referral Network. Below is an overview of the project as well as the information that is required from your organization.

Simplifying access to community services.
Navigating the complicated health care system is challenging. It is difficult to understand what organization provides which services, where, and whether a person is eligible for service. Connecting to these services can be even harder, and people are often left wondering who is going to help them, and when.
Archipel is bringing organizations together to transform the way that the public, primary care, clinicians and community service providers search for services and access care.
By leveraging available digital health tools available in the region, Archipel will be launching a new website with a 24/7 service navigation and eReferral portal, making it easy for anyone to find local, trusted services and send a service request. A secure back-end interface will be available for Archipel partner organizations to receive and manage inbound requests and publish available appointment times.
As a trusted partner of the Kawartha Lakes Ontario Health Team (KLOHT), you've been invited to join the KLOHT Referral Network. This page will share more about the project and how to get involved.
Project scope
In terms of navigation, the target population includes people whose life and health circumstances make them particularly vulnerable in terms of their ability or capacity to identify the best care, services and resources and to understand where and how to obtain/access them. The circumstances of people in vulnerable situations may include (but are not limited to):
- A decline in physical, mental and cognitive health
- Lack of informal care/support
- No access (or difficult/limited access) to a primary care provider
- Difficulty understanding and accessing the multitude of appropriate health and social programs and services
- Without access (or difficult access) to resources and computerized access systems
- Problematic determinants of health

What is Caredove?
The Archipel network platform is built on Caredove.
Caredove is an Ontario-based eReferral management platform that creates trusted networks of home and community services. Clinicians can search for services, book appointments and manage eReferrals within Caredove. Caredove also builds secure websites that help 800+ organizations market their services and receive public requests through a responsive eReferral portal.

Signing up to Caredove is simple.
If you are new to Caredove, the onboarding & training process takes approximately 1-2 hours per organization.
- Fill out the form with your basic organization information.
- Meet with Tina, the Caredove Operations expert who will meet with you virtually to get you familiar with Caredove.
- Learn how to create service listings, build an intake appointment schedule and manage your inbound referrals.
If you are already using Caredove to receive referrals, you may not require any additional training. Tina will be in touch and answer any questions you may have.
